The City of Camden, New Jersey is seeking a social service/regional healthcare provider to facilitate and provide comprehensive health and related social support services to Camden City residents who are experiencing hypertension and type 2 diabetes as underlining health issues relative to high exposure to the COVID 19 virus.  The chosen healthcare provider would also provide training to expand and/or introduce a pool of health care workers to service our residents directly in the community. It is expected that this provider will develop and implement a community based, healthcare delivery model that will address the above health inequities inclusive of other ancillary social, physical, and psychological issues affecting the city’s most vulnerable population.
